Job Description:
A company is looking for a Care Manager (RN).
Key Responsibilities
Develops and facilitates complex care management activities and
personalized care plans for members
Coordinates care between members, families, and healthcare
providers to ensure adequate services
Monitors member progress and updates care plans as necessary to
meet goals and needs
Required Qualifications
Degree from an Accredited School of Nursing or Bachelor's degree in
Nursing
2 - 4 years of related experience
Registered Nurse (RN) State Licensure and/or Compact State
Licensure required
Experience in home health or case management preferred
Ability to work remotely within the Central Time Zone
